Guidelines for International patients

How to apple

International patients who want to make a Reservation Clinic/Second Opinion Service appointment should always apply through a medical coordinator. Please note that we cannot accept applications from individuals. If you do not yet have a medical coordinator, please see the List of Registered Guarantors approved by the Ministry of Foreign Affairs.

Please note that in some cases it may take time to arrange a time and date for an appointment. We may also be unable to offer the second opinion service if the information provided by the patient is insufficient or incomplete.

Necessary documents

We also ask international patients to provide copies of their passport and Japanese visa. Please have them ready together with the following documents.

  • 1.Application Form
    Note: Please complete in Japanese.
  • 2.Medical Information Provision Form in your country ( Japanese translation only)
  • 3.Pertinent test results (Japanese translation only)
  • 4.Copy of patient’s passport (page with photograph)
  • 5.If you have digital images, please send a CD-ROM (DICOM format) by postal mail.

※※Please come to your appointment and together with a medical interpreter if patients who cannot speak Japanese.

Second Opinion Service times and fees

Within 30 minutes: 22,000yen (tax included)
Addition of 11,000 yen every 15 minutes thereafter

Fees

※ International patients who do not have Japanese public health insurance are charged at a rate of 20 yen per medical remuneration point (all patients with Japanese public health insurance are charged at a rate of 10 yen per medical remuneration point).
※ Cash payment or the following credit cards are acceptable:VISA, Mastercard, JCB, , 銀聯(Union Pay)
